GLPs Ozempic, Wegovy and Mounjaro can be life-changing. But if you push them too far, they can backfire.
The Myth:
The Reality
Starvation mode is real. When your body thinks it’s in famine, it slows everything down to survive. In the Minnesota Starvation Experiment (Keys et al., 1950), men on 1,500 kcal/day lost ~25% of body weight and developed classic “starvation mode” symptoms: fatigue, cold intolerance, food obsession, hair loss, low mood.
In The Biggest Loser study, contestants lost huge amounts of weight through extreme diet + exercise.
At the end, their resting metabolic rate (RMR) had dropped by ~610 kcal/day below what would be expected for their size.
6 years later, even though most regained weight, their RMR was still suppressed.
This shows the body can maintain a long-term “starvation response,” making weight regain more likely.
This is not just adaptive thermogenesis in the short term — it’s a persistent metabolic brake that feels very real to patients.
Common Symptoms
- Persistent fatigue & low energy – metabolism slows to conserve fuel.
- Feeling cold all the time – reduced heat production.
- Hair shedding, brittle nails, dry skin – poor protein and micronutrient intake.
- Constipation & bloating – slower gut motility.
- Dizziness & low blood pressure – reduced circulation and electrolytes.
- Menstrual irregularities or low libido – reproductive hormones shut down.
- Brain fog, poor focus, irritability, low mood – brain signalling affected by low energy.
- Muscle weakness & loss of strength – lean tissue is broken down for fuel.
These are the body’s warning lights that you’re running on empty.
How GLP-1s Can Trigger It
GLP-1s lower appetite. Used sensibly, they help you eat less without constant hunger.
But if the dose is pushed too high:
- You may barely eat anything
- Protein intake collapses hence faster muscle loss
- Metabolism slows sharply
- Symptoms of starvation mode appear, even though you don’t feel hungry
This is why some people on GLP-1s report: fatigue, cold hands/feet, hair loss, constipation, loss of periods, low mood. These are all classic signs of semi-starvation.
The Safe Approach
- Stay on the lowest dose that controls food noise (more is not better)
- Prioritise protein at every meal (to protect muscle + metabolism)
- Add strength training to hold onto lean mass
- Keep an eye on symptoms: they’re signals, not nuisances
GLPs don’t automatically put you in starvation mode. But if you push the dose too high and stop eating properly, you can end up there, with fatigue, cold intolerance, hair loss, constipation, hormonal disruption, and a metabolism that grinds to a halt.
Use GLPs as a tool, not a crash diet. The goal is long-term health, not just fast weight loss.
